Insurance Coverage for Acupuncture and Naturopathic Care in Washington State
This information applies to Zen Attitude Acupuncture in Washington.

In-Network Insurance Plans
We are in-network with most major health insurance providers, including:
- All PIP / MVA / Car Accident Insurances — such as Progressive, State Farm, Allstate, Geico, Liberty Mutual, Safeco, and AMF.
- Most Blue Cross / Blue Shield networks and states — including Anthem and Empire plans.
If your insurance plan isn’t listed, please call or text us at (206) 466-2933 to confirm your coverage.
Out-of-Network or Non-Participating Plans
We are not in-network with the following insurance plans:
- Multiplan and Cigna (discontinued since 2018)
- UHC Community Plan
- Most Medicare plans (we do accept Humana and Kaiser Medicare)
- Medicaid (including Molina, Amerigroup, and Apple Health)
- EPO plans within Blue Cross/Blue Shield
Understanding Your Acupuncture Benefits
Even if we are in-network, coverage varies between plans.
As a courtesy, our team can check your benefits, but please note that this is not a guarantee of coverage.
Insurance can be confusing — but we want to empower you to make informed decisions!
Here are key questions to ask your insurance provider when verifying your benefits:
- Zen Attitude Acupuncture says they’re in-network — does my plan show them as in-network too?
- Do I have acupuncture benefits?
- Are acupuncture benefits combined with chiropractic or massage services?
- Do I have a deductible, and does it apply to these benefits?
- Is there a limit to the number of visits?
- Do I owe a co-pay or co-insurance?
- Is there a separate co-pay or co-insurance for a new patient visit (CPT code 99203)?
- Do I need pre-authorization before starting treatment?
- Have I met my out-of-pocket maximum for the year?
- (Premera patients) Do I have a separate co-pay for cupping therapy?
✅ Tip: Always ask for the representative’s name and reference number at the end of your call.
Out-of-Network and Self-Pay Options
If your insurance is not listed or you prefer not to use your insurance, you may still be eligible for:
- Out-of-Network Reimbursement – Some insurance plans cover a portion of the visit even if we are not in-network. Contact your provider to check your out-of-network benefits.
- Discounted Cash Rates – Available for patients with Medicare, Medicaid, or no insurance coverage. These rates are only valid if agreed upon with your provider and paid on the date of service.
- A valid photo ID is required for all patients, including those paying out of pocket.
Please note: Massage therapy services are not in-network with most insurance plans. For more details, contact your local Zen Attitude office.
